Printing instructions

If you are familiar with your browser's printing habits and settings and are confident of the result, you can just go to the printable page and click the button. You want the form to all print out on one page.

Otherwise. . .

If you are using IE you should check your browser's print settings before printing and possibly wasting ink, paper and your time. Go to the Printable Version, click on File > Print Preview. Click on the Page Settings Icon (see cursor position in second screenshot) and check those settings. The page printed correctly on my own computer with the settings you see, your own may vary. Once you have the settings the way you want, close and check to make sure that the preview is now showing {page 1 of 1} at top and then click "Print".

If you are using FireFox you will likely have to adjust your browser's printing to get it to print the whole document on one page. We suggest you use the "File > Print Preview" in Firefox to get it set correctly to print to just one page before trying to print and possibly wasting your time, ink and paper. You can probably accomplish this by either of two methods:

  1. Check the box for "shrink to fit" on the "Scale" drop-down menu of the "Print Preview" window.
  2. Change the margins and header settings on the "Margins and Header/Footer" tab in the "Page Setup" window. See below for screenshots.
